It’s true: we are a cash-based practice because we believe in transparency. We are honest with patients about costs for our medical services, so you can decide for yourself whether it is worth it before receiving care, as opposed to insurance-based practices where you get sticker-shocked after services have already been rendered and you have no choice but to pay .
- HSAs (Health Savings Accounts ), FSAs (Flexible Spending Accounts) and HRAs (Health Reimbursement Accounts) are welcome!
- We are out-of-network for all commercial health plans (e.g., Aetna, BCBS, Harvard Pilgrim, Tufts) but can provide claim slips (“superbills”) you may submit directly to your plan to seek reimbursement.
- Membership with installment plans and Pay-As-You-Go options are available
- We are unfortunately only enrolling new Medicare patients under very special circumstances. We are deeply sorry for the inconvenience. If you have Medicare and are still comfortable with what we charge for consultation, please call us to discuss options.

Notes on Medicare and Telehealth
I regret to report that effective April 1, 2025, Medicare will no longer cover Telehealth visits except for very specific clinical encounters that are not applicable in the Primary Care setting.
What this means for us at Thrive Adult Primary Care is that to stay solvent, we will only be able to offer the convenience of Telehealth encounters by charging out of pocket fees. Medicare patients who are not paying a concierge membership fee will only have coverage (through Medicare) for direct physician access in the setting of in-person encounters.
After April 1, text or email communication will only be accepted for renewals of standing medication orders. ANY changes or other queries that entail medical decision-making, will either need to be addressed in the office, or will incur out-of-pocket fees as detailed below.
NO CHANGES are being made to the fee schedule for Direct Primary Care members or pricing of Pay-As-You-Go services. Medicare patients who have a TranscEnd Concierge Membership (paid monthly or annually) will also continue to enjoy physician access via any/all remote measures with NO CHANGES in fees at this time.
NOTE: Patients with any type of PAID MEMBERSHIP do NOT have to pay for any Telehealth encounters.
